Approve and Adjust Radiation Worker Form - Academic Line Manager

Steps to complete

  1. Navigate to the form via the email
  2. Review the worker's information
  3. If there are any discrepancies in the form, note them in the Comments box
  4. Sign the form
  5. Select the Submit button at the top of the page
     

This video explains the process for terminating a radiation worker's registration when it’s unjustified or unnecessary, and ensuring visibility for the RPS, SRPS, and the Safety Office. Please note terminations are irreversible. The steps that need completing are outlined below.

Termination of a Radiation Worker Registration Form - Academic Line Manager

Steps to complete

  1. Navigate to the form via the email
  2. Select the Terminate Workflow button at the top of the form
  3. Enter a reason for terminating the form
  4. Select Terminate
     

This video explains how to authorise new work for an existing radiation worker using the Work Type Registration form. You can review and note any discrepancies, approve or terminate the form. As with the Worker Registration, terminated forms are irreversible. The steps that need completing for both approving and rejecting the form are outlined below.

Steps to complete

  1. Navigate to the form using the email you have received
  2. Complete all the mandatory fields marked with an asterisk. If you are a visitor you will also need to upload an approval document from your employer. Note: there are some fields that will be prepopulated and cannot be changed
  3. Read through the declaration
  4. Sign the form
  5. Select the Submit button at the top of the page
