Incident investigation arrangements

Accident and incident investigations are an important part of the 'check' process of health and safety management. They take place after the immediate actions necessary to make an area safe following an incident.

The aim of an investigation is to establish the immediate, underlying and root causes of an accident or incident. This helps identify actions that will prevent reoccurence. Investigations are not intended to apportion blame; the University operates a no blame policy for reporting and investigating accidents and incidents.

Please Note: Accidents and Incidents

For expediency, the term accidents and incidents is used to encompass accidents, incidents, dangerous occurrences, specified occupational exposures, ill-health incidents, environmental incidents, and near misses.
